This standard release form is required for each participant in St. Paul’s Children and Youth Ministries.

St. Paul’s Youth Participation Covenant

This covenant is required for each participant in grades 7 to 12. This helps provide for our youth and chaperones’ physical, emotional, and spiritual safety.
